I found the problem. It was either permissions or ownership. I think it was the permissions on the quiz_install.php file. I changed the permissions and changed the ownership to 755 and it worked.

I have followed the file moves and changes with no problems, but when I go to run the quiz_install.php file I get a blank screen.
